Medical, Transportation, & Liability Release:
BY CHECKING THE BOX BELOW, YOU AGREE TO THE FOLLOWING: SICK POLICY: I give the Camp Nurse permission to administer Over-the-Counter medicine if needed to treat aches, pains, bumps, scratches, etc. If a registered attendee develops a fever during their time at Sunset, they will be immediately isolated until they can be taken home. No refunds are given because of early departure due to sicknesss. PLEASE DO NOT KNOWINGLY SEND A CAMPER TO CAMP WITH ANY ILLNESS OR INFECTION LIKE THE FLU OR STOMACH BUG.
EMERGENCY TREATMENT: I hereby give consent for Sunset Bible Camp (Camp) to obtain or administer proper medical treatment to the registered attendee in the event of illness or medical emergency which may occur during their stay at Camp, and will in no way hold Camp management, staff, or volunteers responsible for any accidents caused by negligence or disobedience on the part of the Campers. This application has my approval for the child to participate in all Camp activities and be bound by all Camp policies in force.
PHOTO RELEASE: When registering at Sunset Bible Camp, you are authorizing Sunset Bible Camp the use and reproduction of any and all photographs, video or film taken of the person whose record you are updating during program activities. There will be no compensation to you. All negatives and positives, together with said prints, video or film are the Property of Sunset Bible Camp, solely and completely. You also waive any right to inspect or approve any photo, video or film taken during your visit. I affirmatively release and discharge Sunset Bible Camp from any responsibility for any distortion or manipulation, whether intentional or otherwise, of photos, video or film taken of my person or registrant.
TRAVEL RELEASE:By checking below, I also agree to allow my camper to be transported by Pathway Christian Church to Sunset Bible Camp by Church Van.
Non Refundable deposit of $50 is due to register your student for camp.
Remaining $80 is due by April 29th, any registraions after this date will incure a $25 late fee.
